On Xiaochaihu decoction
Clinically, I always like to use classic prescriptions, and the most commonly used ones are Xiaochaihu Decoction and its modified and integrated prescriptions. The so-called modified prescriptions include five kinds of Bupleurum soups in Treatise on Febrile Diseases: Bupleurum Plus Gulong Oyster Soup, Dachaihu Soup, Chaihu Guizhi Soup, Chaihu Plus Mirabilite Soup and Chaihu Guizhi Ganjiang Soup.

In addition, Xiaochaihu Decoction is compatible with Wei Ping Powder, Wendan Decoction and Wuling Powder, and was named as Jiejing Formula by Mr. Liu Duzhou. These two kinds of prescriptions are not only widely used in exogenous diseases, but also widely used in internal medicine, gynecology and pediatrics, with remarkable curative effects, and are indeed worthy of being the "elite troops" in the classics.

Many ancient and modern medical sages have made a very detailed study on Xiaochaihu Decoction. Let's take a look at Mr. Liu Weihong's analysis of the functions of the seven herbs in this prescription:

① Shugan (Bupleurum); ② Clearing gallbladder (Scutellaria baicalensis Georgi); ③ Harmony with stomach (Pinellia ternata and ginger); 4 spleen (ginseng, jujube, roasted licorice).

The related symptoms are also divided into four categories:

① Stagnation of liver qi: alternating cold and heat, fullness in chest and hypochondrium, silent expression, and thready pulse. ② stagnation of gallbladder and fu organs: bitter taste, dry throat, dizziness and upset. ③ Disharmony between stomach and qi: nausea. 4 weak temper: I don't want to eat.

In this way, the outline card and "four main cards" of Xiaochaihu decoction are clearly organized, which makes people feel relieved at a glance.

Xiaochaihu decoction is not only suitable for the syndromes of "liver depression, gallbladder heat, epigastrium and spleen deficiency", but also its modified prescriptions (including the combination of classic prescriptions and classic prescriptions, and the combination of classic prescriptions and time prescriptions) expand its application scope. Mr. Liu Duzhou said: "Advocating and popularizing Bupleurum is the first party to connect ancient and modern times." Fangshan is famous for treating major crises and incurable diseases. The following are two typical medical records of my application of Chaihu Decoction:

1. Dachaihu decoction in the treatment of suspected acute pancreatitis

Huang Moumou, male, 32 years old, farmer entrepreneur. At noon on February 20th 1993, due to overeating, chest tightness, abdominal distension and paroxysmal abdominal pain occurred. I didn't mind at first. I will have an infusion at the local medical station. Unexpectedly, two days later, his condition went from bad to worse, with aversion to cold and high fever (T39.3℃), shortness of breath, aggravated chest and hypochondriac pain, cyanosis of lips and black claws, and he was rushed to our hospital for medical treatment.

After consultation, it was diagnosed as: ① acute infection of chronic cholecystitis; ② Acute gastritis; ③ Acute peritonitis? ④ Acute pancreatitis?

At that time, the condition was already critical, but the biochemical examination was quite different. X-ray abdomen showed no abnormality, and blood routine was normal except white blood cells1.4000 units. Blood and urine amylase tests are within the normal range.

At that time, in addition to oxygen inhalation and nasal feeding tube (to reduce abdominal pressure), patients were also given cefradine, ampicillin and other intravenous drip anti-inflammatory. However, the condition is getting worse. In addition to the critically ill notice, if you want to refer to Guangzhou, your family members and attending doctors are afraid that the road will be uneven, and the patient can't stand the 8-hour bump and is in a dilemma. I was invited for urgent consultation.

Engraving: acute and painful, shortness of breath, abdominal distension as full as a drum, obvious tenderness in the upper abdomen, rebound pain in the lower abdomen, persistent fever, especially in the afternoon, T39.2℃, loose stool for three days, a small amount of urinary catheter, slippery pulse (96 times/minute), red tongue and thin white fur.

Seeing this syndrome, it is obvious that Shaoyang and Yangming are co-diseases, that is, the proposed Dachaihu Decoction is modified: Bupleurum 20g, Scutellaria baicalensis Georgi 12g, Fructus Aurantii 15g, Radix Paeoniae Alba 18g, Magnolia Officinalis 15g, Faxia 10g, and Rhubarb/kloc. One dose, decoction.

One and a half hours after taking the upper part, sticky yellow urine is released first, and then foul mucus is discharged, which is stinking. Chest tightness, abdominal distension, hypochondriac pain obviously subsided, the body temperature gradually dropped to 37℃, the spirit improved, the pulse was slightly slippery (82 beats/min), the tongue was red and the fur was thin and white, and the illness turned to safety. Therefore, the nasal feeding tube and oxygen tube were removed. I'll add and subtract Houpu Sanwu Decoction and Dahuang Mudan Skin Decoction, and I'll be discharged after one week.

This syndrome can be called critical. Western medicine consultation is most likely to be acute pancreatitis and acute peritonitis, but it does not support laboratory biochemical examination and the treatment is ineffective. The prescription was changed to traditional Chinese medicine, and the six meridians were differentiated, and the prescription was treated, and finally all the work was achieved.

2. Chaihu Guizhi Decoction is used to treat diseases of sun and shaoyang.

Li's wife, a 40-year-old peasant woman, is from Shajiaowei Village, Jiesheng, this city. It was first diagnosed in September 2000. Two months ago, the patient suffered from exogenous diseases. At that time, he had a fever, aversion to cold and joint pain all over his body. He was treated as rheumatism by local doctors and Pengpai Hospital with repeated symptoms. Because of poverty, the cattle on the farm were sold, but they have not recovered. One of my students was from the village at that time. Seeing her pity, she was introduced to my office for treatment.

I can see that my face is tired, complaining that I still have a fever, aversion to cold, bitter taste, dizziness, pain on both sides and joint pain in my limbs every afternoon. I can eat, sleep and defecate. The tongue coating is yellow and the pulse is thin and string. This syndrome is consistent with the syndrome of Chaihu Guizhi Decoction described in Treatise on Febrile Diseases (slight aversion to cold due to fever, vexation and pain of branches and joints, slight vomiting, and branching knot under the heart), which means that the sun is deficient in yang, and it is an improved version of Chaihu Guizhi Decoction;

Bupleurum 15g, Scutellaria baicalensis 12g, Faxia 10g, Ramulus Cinnamomi 6g, Radix Paeoniae Alba 12g, Radix Pseudostellariae 15g, Fructus Jujubae 20g, Radix Glycyrrhizae 3g, Radix Stephaniae Tetrandrae 10g, and Rhizoma Zingiberis Recens.

Five doses.

Only take more than 1 dose, if all symptoms disappear; After taking 5 doses, the man has recovered. The whole family was very happy and invited my students to thank them.

Some people think that some medical records claim that drugs are effective in treating diseases, but such cases are not exaggerated. Mr. Liu Duzhou called the combination of classic prescription and modern prescription the combination of ancient and modern. This kind of integration is to make up for the weakness of "Shi Fang" with "Gu Fang", and to make up for the imperfection of "Gu Fang" with "Shi Fang".

3. Xiaochaihu decoction combined with Sanmiao powder to treat exogenous dizziness and bitter taste.

About the summer of 1992, an exogenous patient came to see a doctor. From the symptoms, he is dizzy, dry mouth, bitter taste, sometimes aversion to cold, Huang Chi in urine, red tongue and thready pulse. The private prosecution has changed three doctors, which lasted for four weeks, and they all planned to see a doctor because the treatment of exogenous diseases was ineffective.

I'm going to add tribulus terrestris and Bombyx Batryticatus to Xiaochaihu soup, and I'm secretly pleased. I'm sure three doses will succeed. However, things are unpredictable, and the patient actually came back for a visit. Dizziness and aversion to cold have eased, and the remaining symptoms are still the same. After careful examination, it was found that there was yellow greasy and slightly thick fur in the posterior segment of the tongue, accompanied by damp heat in the lower coke. No wonder he peed in the yellow pool. So he added Sanmiao Powder: Cortex Phellodendri 12g, Rhizoma Atractylodis 6g and Coicis Semen 20g, and three doses were cured.

4. Bupleurum chinense and Wei Ping powder.

Some patients have damp-heat in the original spleen and stomach, with symptoms such as bad breath, yellow greasy tongue coating or yellow greasy urine, sticky stool and so on. Once exogenous diseases appear, it is useless to simply dispel cold or wind and heat. I often add and subtract Xiaochaihu decoction and Huanglian Wei Ping powder, and I often finish three or five doses.

For those with cough and phlegm due to cold and heat, relieving exterior syndrome is the main treatment, and relieving cough and resolving phlegm is the auxiliary treatment. At this time, my killer is Xiaochaihu decoction combined with San 'ao decoction, and then according to cold and heat, Erchen decoction or Scutellaria baicalensis Georgi, Mulberry bark and loquat leaf, which can clear lung heat, are appropriately added and subtracted according to syndromes, which often get twice the result with half the effort.

I once wrote a paper "Clinical Application Experience of Bupleurum and Scutellaria baicalensis Georgi in Treating Infectious Fever", affirming that Chai and Qin not only treat exogenous fever, but also treat infectious fever, and verifying the antipyretic effect of Xiaochaihu Decoction, which is summarized as five points:

"Although there are dozens of syndromes in Treatise on Febrile Diseases, there are five syndromes that are the most appropriate, and they will be cured after taking them.

People with body heat, heart attack or vomiting can take it. Typhoid fever is the most common syndrome, so Xiaochaihu decoction should be taken properly. If you vomit because of thirst, please don't take it; Please don't take it if you are not warm. This should still be known.

Both those who are afraid of cold and those who are hot and cold can take it.

The three are damp and hot.

All four can be taken when you are upset and full, and when you are thirsty.

After typhoid fever has passed, people with more fever can take it.

These five cards: But if you have a card, you can take it without a doubt, and you must take it. "Honesty is not far off the mark!

Now I also talk about the clinical application of six prescriptions named Bupleurum in Treatise on Febrile Diseases, and talk about some superficial clinical experiences on the compatibility of Xiaochaihu decoction with ten prescriptions. I think Chaihu decoction has many opportunities for clinical application, and it is used almost every day. No wonder some people say that learning to use Chaihu decoction can cope with more than half of clinical diseases.
