Last month, when I went to Beijing, I was lucky enough to visit the disease-free teachers in the world. It was his book "Learning Chinese Medicine from scratch" that let me enter the door of learning Chinese medicine. The teacher told me that the study of Chinese medicine is a process of combining brain, mind and body. We remember knowledge with our brains, feel and perceive with our hearts, and practice with our bodies to achieve proficiency. Especially the role of the heart, let us experience the experience taught by the ancients through two connected feelings. For example, in pulse diagnosis, the pulse of Qi (kūu) is wide and has a weak feeling in the middle, like fingers touching the onion tube. So we quickly realized and remembered this pulse condition and pulse condition. Therefore, the teacher talked about the introductory method of Chinese medicine from the beginning, that is, "treating with the body and measuring with heart." Thus, 12 TCM experience and lessons were introduced:
The first lecture on understanding traditional Chinese medicine
The Second Mystery of TCM Thought —— Yin-Yang and Five Elements
The third lecture on the mysteries of human physiology-Zang-fu meridians
The fourth lecture on the mystery of human pathology-the debate between good and evil
The fifth lecture on the mystery of TCM diagnosis-looking, listening and asking
The sixth lecture on the mystery of dialectics of traditional Chinese medicine-dialectics of eight outlines
The seventh lecture on the mystery of nature and taste of traditional Chinese medicine-four qi and five flavors
Lecture 8 The Mystery of Compatibility of Traditional Chinese Medicine —— Jun Chen Zuo's Family
Lecture 9 The Mystery of Famous Doctors' Achievements —— Interpretation of Medical Records
Lecture 10 The Mystery of Acupuncture and Moxibustion-Meridian Principle
Lecture 11 The Mystery of Acupuncture-Meridian Differentiation and Treatment
Lecture 12 Re-understanding Chinese Medicine
Through a thin page 180, the disease-free teacher followed the clues of theory, law, prescription, medicine, case and needle, and used the easy-to-understand method to step by step, highlight the key points and echo the cases before and after. It has a unique charm by describing "the taste of yin and yang, the image of viscera, the difference between good and evil, the subtlety of diagnosis, the nature of prescriptions and the nature of acupuncture". Especially the dialectical chapter, which is a hundred words, integrates the dialectical outline of zang-fu organs, echoes before and after, and is the finishing touch of the book. "
In the sixth chapter of Dialectics of the Eight Outline, Mr. Wu Bing summarized his previous teaching experience into a set of easy-to-understand dialectical methods, and briefly introduced them to you here:
Distinguish the nature of the disease first, and then distinguish the location of the disease. The nature of the disease is deficiency and excess, and the disease is located in the five internal organs.
Key points of deficiency syndrome:
Qi deficiency, fatigue, laziness, blood deficiency, lethargy and shallow dreams;
Yin deficiency, small and thin tongue, night sweat, yang deficiency, cold and weakness.
Qi deficiency: fatigue, laziness and weak pulse;
Blood deficiency: female insomnia (dreaminess), dry and painful eyes, less menstrual flow and pale face;
Yin deficiency: hot flashes, night sweats, fever due to five senses, red tongue, emaciation, scanty coating and fluid, and thready pulse;
Yang deficiency: aversion to cold and warmth, cold limbs and loose stool, long urine, pale tongue, excessive body fluid and slow pulse.
Key points of experience:
Hot red and yellow, cold and cool, phlegm and dampness, heavy tongue, sticky;
The wind is skewed and active, while qi stagnation is blood stasis and purple darkness.
Cold syndrome: cold form, cold limbs, perennial tepid, and delayed pulse;
Heat syndrome: red face, body heat, upset, red tongue, yellow tongue coating and rapid pulse;
Phlegm-dampness syndrome: top-heavy, sticky stool, greasy tongue coating;
Dryness syndrome: dry mouth and dry stool;
Wind syndrome: tremor, skew and channeling;
Qi stagnation: chest tightness, abdominal distension, fullness under the ribs, sighing;
Blood stasis: dark purple lips, ecchymosis or ecchymosis on the tongue, and thick purple collaterals under the tongue.
Key points of disease localization:
Lung disease is nothing more than expectoration and asthma, heart disease, palpitation, pain and insomnia;
Spleen disease reduces digestive appetite, abdominal distension and pain, and makes defecation difficult;
The liver is full of vital energy, and the kidney has difficulty urinating in the waist and knees.
Lung: cough, asthma, phlegm;
Heart: palpitation (palpitation), insomnia, heartache;
Spleen: anorexia, abdominal distension, abdominal pain, diarrhea and constipation;
Liver: distension (under the rib), pulse string;
Kidney: weakness of waist and knees, abnormal urination and sexual dysfunction.
Teacher Wu Bing leads the readers to carry out the most basic thinking training through the "hundred-word formula" combined with case analysis. All future flexibility needs to be based on this. Starting from this solidified mode, it is a shortcut to learn clinical thinking of traditional Chinese medicine.
Let's take the example of "oral ulcer" in the book:
Li, male, 54 years old, cadre, recurrent aphtha 1 year. First visit on May 6th, 2002, it was observed that most of the prescriptions used before were mainly clearing away heat and purging fire, nourishing yin and reducing fire. At present, the patient's face is dark yellow, and there are several ulcerated faces the size of mung beans on his lips and cheeks. The color is white with yellow, and the surrounding is reddish and not swollen. He claimed that he suffered from pain when eating, accompanied by dry mouth, no desire to drink, and he was sleepy all over. Sometimes he had loose stool, poor sleep, red tongue, thick white fur and weak pulse. Questioning the medical history, the patient suffered from heavy work pressure and frequent insomnia in recent 1 year.
The disease-free teacher wrote: aphtha is incurable for a long time and recurs to the author, which is mostly related to liver, spleen and kidney. Long-term liver depression and qi stagnation, fire from endogenous, can go up along the stomach meridian, resulting in aphtha; Deficiency of temper, inability to promote, stagnation of phlegm and dampness diet can also lead to behavioral diseases; If yang deficiency is insufficient, kidney yang is insufficient, and fire is not enough to warm spleen and soil, you can also see the typical performance of upper heat and lower cold, thus producing aphtha.
Dialectical process of disease-free teachers;
1. First of all, look at the main symptoms: "There are several mung bean-sized ulcerated surfaces on the lips and cheeks, which are white and yellow in color, reddish and not swollen around, and they complain of pain when eating." The color of ulcer wound is light but not red, and the swelling and pain are not obvious, suggesting that the disease is mostly deficiency heat rather than excess heat.
2. Look at the complicated disease "dry mouth and don't want to drink". Dry mouth is the lack of water in the upper part for two reasons: first, water is insufficient, or water is consumed because of fierce firepower; Or yin deficiency and fire are endogenous, and water is even less. In both cases, dry mouth means thirst. Second, there is a lot of water, but it can't reach the mouth, mostly because it can't be watered. Although sharbat, there is actually a lot of water. After drinking water, the burden is further aggravated, qi deficiency is getting more and more serious, but I don't want to drink when I am thirsty.
3. Continue to look at the whole body performance: "The whole body is sleepy and weak, sometimes loose stool, poor sleep". Combined with the key points of syndrome differentiation such as "Qi deficiency, fatigue, laziness in speaking, blood deficiency, astringent eyes, shallow dreams", "phlegm and dampness are heavy and sticky", "heart disease, palpitation, pain and insomnia"; Spleen disease makes digestion and appetite decline, abdominal distension and abdominal pain are difficult to defecate. "It can be known that patients are deficient in both qi and blood, mainly qi deficiency; See wet evil; The disease is located in the spleen and heart. Finally, look at the tongue pulse "the tongue is red, the coating is white and thick, and the pulse is thin and weak." The results are exactly the same as those obtained from the previous analysis.
4. The treatment plan should focus on strengthening the spleen and stomach. The doctor used Shen Ling Baizhu Powder to add and subtract (general picture).
5. The whole process focuses on syndrome differentiation, secondary treatment, and then prescription. Clear thinking, in turn, wants to step into the door of Chinese medicine, why is it difficult? By collecting and sorting out the medical records of recurrent oral ulcer, keeping in mind the essentials of syndrome differentiation and analyzing them, we can understand the symptoms of oral ulcer, the main points of diagnosis, how to find the target of treatment and how to use common prescriptions. Clinical problems can also benefit from it.
There are many chapters that are clear, focused and easy to understand, such as "Eight Cardinal Dialectics Hundred Words":
1. Common constitutions include qi deficiency, deficiency of both qi and blood, yin deficiency, yang deficiency and excessive dampness, so it is advisable to avoid suggestions and keep traditional Chinese medicine, Chinese patent medicine and special meridians;
2. 100 Examples of traditional Chinese medicines commonly used for excess syndrome and deficiency syndrome;
3. Three stages of medical learning: primary identification stage, intermediate syndrome differentiation stage and advanced treatment stage;
4. Self-study the required bibliography of Introduction to Traditional Chinese Medicine and so on.
In short, this book is a must-read for the introduction of Chinese medicine, and it can also be used as a reference book to see if the foundation of studying medicine for three or four years is solid. In addition, Wu's new work "Zero Starting Point Acupuncture" is expected to be listed at the end of the year, and Cheng Entropy said that he can't wait.
