Because most patients with type II diabetes have no obvious symptoms, the onset is slow, more than half of them are asymptomatic, and it is not easy to be found in the early stage, so most patients cannot get early treatment. What's more, the existing diabetes detection methods have certain limitations on the early detection of diabetes! Professor Pan Changyu, a famous endocrinologist in China and the General Hospital of the People's Liberation Army, pointed out that only detecting fasting blood glucose will miss the diagnosis of nearly 80% of pre-diabetes and diabetic patients. Because when the fasting blood glucose is 6. 1 mmol/L, it may be a diabetic. Therefore, when patients are diagnosed with diabetes, they are often found to have serious complications, such as stroke, coronary heart disease, eye disease, diabetic nephropathy, dermatosis or gangrene of lower limbs, and have a medical history of at least 5- 10 years.

In addition to medication, the behavior and self-management ability of diabetic patients are also the key to the success of diabetes control. Therefore, the control of diabetes is not a traditional treatment, but a systematic management. In order to achieve this goal, a relatively perfect diabetes education and management system should be established. According to the "Management Standard of Whole-course Nursing Network for Diabetic Patients in xx City (Trial)", combined with the actual situation in our district, the "Implementation Plan of Whole-course Nursing Network for Diabetic Patients in xx District" was formulated.
First, the management mode of "Sugar Friends Network"

3. Quality control management: The specialists or experts of the Second Hospital of xx City shall formulate the management path, process and goal of patients accessing Sugar Friends Network. Training mechanism: The District Health Bureau and the Second Hospital of xx should undertake professional practice guidance and related training for general practitioners, nurses and managers, and organize various trainings organized by the Municipal Health Bureau.
