According to the requirements of Henan Education Examinations Institute, all candidates taking the exam must complete a health report before noon 12 every day. The deadline for completing the form is 12:00 on the day of the end of the exam, and each person can only complete the form once a day, and cannot repeat the form.

First, download and install.

1. Android users download the APP: search for "health report" through the mobile app market, or download and install it.

3. After the download is completed, the "Health Report" icon will be automatically generated on the mobile phone desktop. Click the icon to enter the login interface, enter the candidate number, password and verification code, and click the login button to enter the punching interface. Note: If the verification code is unclear, you can click the verification code to refresh.

Second, authorize the application authority.

After successful login, you will enter the report page, and you will apply for permission when you enter for the first time. Please be sure to allow the application to use the location permission, otherwise it will lead to the failure of location and the subsequent process can not be carried out normally, so please be sure to agree to the application for permission. If you can't get the location due to network reasons or other circumstances, you can try to get it manually by clicking "Get Location". Please check whether the GPS positioning function of the mobile phone is turned on before obtaining it.

Third, candidates' health punching

Get Location: Click Get Location to make sure the location is correct.

Note: If you are in a city other than the test center, a "warm reminder" will pop up. Please read carefully so as not to delay the exam.

Fourth, health status.

Select [Healthy] or [Cough or body temperature higher than 37.3℃] or [Other] for current health status;

From 0/4 day before the exam/kloc-whether to stop or pass by in overseas or domestic high-risk areas, or whether to contact people in high-risk areas, select [Yes] or [No];

Whether you have had contact with suspected or diagnosed COVID-19 patients, choose Yes or No;

Current health code is [green code] or [yellow code] or [red team];
