Etiology and pathogenesis of spontaneous sweating and night sweats

Spontaneous sweating and night sweats are actually common symptoms in clinical miscellaneous diseases, which often appear together with palpitation, insomnia, dizziness and tinnitus. It also often appears in the pathogenesis of fatigue, tuberculosis, hemorrhagic syndrome and postpartum blood deficiency of women.

The causes of this disease can be summarized as follows:

(1) from the fiery qi between heaven and earth;

(2) Internal injury of seven emotions leads to depressed fire;

3 fornication, overwork, hurting yin and yang;

(4) improper diet and spicy drinking;

⑤ Overdose or misuse of irritant drugs;

⑥ Loss of blood essence, yin injury due to serious illness, and yang deficiency;

There are many reasons for night sweats.

Zhang Jingyue said, "In case of trouble and fever, you will sweat at most. Therefore, the fire of diet ... fatigue ... wine and color ... all make people sweat. "

In terms of pathogenesis, Jing Yue thinks that spontaneous sweating belongs to the weakness of defensive qi, and night sweats belong to Yang Qi's hyperactivity. If the cause is not solid, defensive qi is also weak, which leads to spontaneous sweating and yin deficiency leads to yang deficiency, so yang steaming leads to blood heat, and blood heat leads to the discharge of body fluid, which is called night sweats.

It can be seen that spontaneous sweating and night sweats are caused by two reasons: yang deficiency and yin deficiency. Either "yin is not observed" or "yang deficiency is not solid", which always leads to imbalance between yin and yang.

Related viscera of spontaneous sweating and night sweats.

"Sweat is the heart fluid and kidney governs five fluids", which is a symptom of spontaneous sweating and night sweats, and most of them are diseases of heart and kidney.

"Su Wen Xuan Ming Wu Qi pian" says: "The five internal organs transform fluid, and the heart is sweat". Zhang Zhicong explained that "the heart governs blood, and sweat is the body fluid of the heart", indicating that sweat and blood are in the same strain.

Su Wen Shui Re Xue Lun said, "Courage and hard work make you sweat". Su Wen's Second Discussion on Dirty Qi Law says: "People with kidney disease ... sleep, sweat and hate the wind". It shows that pathological sweating is often directly related to heart and kidney.

"Lingshu Zangfu Disease Syndrome" says: "Lung pulse ... is very slow and sweaty." "Lingshu Ben Cang Pian" says that "those who defend qi, therefore, warm the meat, make up the skin, make it fat, and those who open and close it". Ye said: "Lung governs qi and belongs to Wei". It can be seen that the formation of spontaneous sweating is also closely related to the strength of lung qi and the rise and fall of Wei qi.

"Su Wen's Theory of Meridian Differentiation" also mentioned: "Surprise and seize the essence, sweat comes from the heart; Keep heavy and walk far, sweating from the kidney; Escape from fear and sweat out of the liver; The waist is hard and sweat comes from the spleen. "

This shows that spontaneous sweating and night sweats are directly related to heart and kidney, lung, spleen (stomach) and other viscera, and also related to defending qi.

Syndrome Differentiation and Treatment of Spontaneous Sweat and Night Sweat

Spontaneous sweating and night sweats have different characteristics in clinic.

Sweating during the day (except in hot summer) is spontaneous sweating. You will sweat when you are asleep, and you will sweat when you slap in the face (except for babies). This is night sweats.

In terms of syndrome differentiation, predecessors have different views. Ru Lin believes that spontaneous sweating belongs to yang deficiency and night sweats belong to yin deficiency. Zhang Jingyue, on the other hand, thinks: "Spontaneous sweating and night sweats also have their own syndromes of yin and yang, so it cannot be said that spontaneous sweating must belong to yang deficiency and night sweats must belong to yin deficiency ... but if there is fire or no fire, it is either yin or none other than yin".

Although the previous theories are different, they are always based on comprehensive syndrome differentiation.

There have been many discussions about the treatment of spontaneous sweating and night sweats. Generally speaking, it is believed that the treatment of yang deficiency should replenish yang, defend the weak and show the exterior; Yin deficiency should nourish yin, and excess heat should clear fire.

In addition, Li Zhongzi put forward the treatment according to the syndrome differentiation of viscera deficiency, arguing that "lung deficiency benefits its fur, heart deficiency benefits its blood, kidney deficiency helps it seal, spleen deficiency benefits qi, and liver deficiency prohibits its discharge".

The law of syndrome differentiation and treatment of spontaneous sweating and night sweats is summarized as follows.

1 Syndrome Differentiation Treatment of Exterior and Interior Yin and Yang

Deficiency of both qi and blood in exterior and interior-excessive sweating, heavy limbs and fatigue-warming interior, benefiting qi and consolidating exterior-Qifutang Ginseng

Lung and stomach are very hot-sudden sweating, body fever, thirst, irritability-clearing heat-Baihutang

In short, the symptoms of spontaneous sweating and night sweats focus on distinguishing between yin and yang and between deficiency and excess. The treatment of yang deficiency is mainly to replenish qi and tonify exterior, while the treatment of yin deficiency is mainly to nourish yin and protect body fluid. If yin and yang are deficient for a long time, yin and yang are complementary.

If the excess and the deficiency are mixed, it is necessary to treat them according to syndrome differentiation. For example, excessive sweating due to yin deficiency and heat can be treated with Danggui Huang Liu Decoction. I think these principles must be mastered in clinic.
