How much do you know about the "duty watch" of internal organs?
Editor's Note Chronology of Traditional Chinese Medicine is an ancient and young theory, and it is a science that studies the influence of time factors on human body by using traditional Chinese medicine theory. The physiological and pathological phenomena of human body should change according to the change of natural rhythm, and the favorable time should be chosen for the treatment and prevention of diseases. Meridian theory is considered as a master of TCM time medicine. It is believed that twelve o'clock every day (every two hours is one hour) is closely related to the the twelve meridians of the human body, and the meridians are matched with the internal organs of the human body, that is, the internal organs are on duty every hour. Application of TCM Chronology in Disease Treatment Coronary Heart Disease: The onset of coronary heart disease has a certain circadian rhythm, with the peak at 6: 00 ~ 12: 00 and 17: 00 ~ 24: 00. Cerebral hemorrhage: Patients with cerebral hemorrhage have the highest incidence in winter at 9: 00 ~1:00 (fourth season) and 17:00~ 19:00 (unitary season). According to the theory of nourishing yin in autumn and winter, patients with cerebral hemorrhage should pay attention to nourishing yin in winter. Fever: Fever caused by frostbite and fever caused by cold-dampness arthralgia are mostly heavy at night and light during the day; Phlegm-qi stagnation syndrome usually occurs at noon (11:00 ~13: 00), but subsides at noon (23: 00 ~ 1: 00). Fever due to yin deficiency generally starts in the afternoon and lasts until the evening. Lung fever, especially at night; Those who are enthusiastic, especially Japanese and China; Spleen fever, especially at night; Liver heat, especially between pubic hair (3:00~7:00). Bronchial asthma: Bronchial asthma is accompanied by "acute onset and nocturnal emission", and the administration time can be adjusted to be taken before going to bed. Insomnia: The alternation of spring, summer, autumn and winter in nature constitutes 365 days a year. The healthy sleep of human body corresponds to nature, and there are also four periods-Shihai, Zishi, Ugly and Yin Shi. These four hours correspond to the rotation of the four seasons, so be sure to sleep well. (1) I can't sleep after waking up in the middle of the night (23:00~ 1:00), which is caused by the imbalance of Yang's ascending axis and Yin and Yang. Xiaochaihu decoction can relieve shaoyang cardinal and correct yin-yang imbalance. (2) After 65438+0:00 am, I can't sleep around 4:00 am. If the onset time is consistent with the pulse condition and symptoms, drugs that calm the liver and benefit the gallbladder can be used. (3) wake up at 3 am to 4 am. Most patients in liver blood deficiency get sick in Yin Shi because they are covered in gold, so they can make use of trees. You can treat it with Suanzaoren Decoction. To sum up, timely treatment may play a multiplier role in the treatment of diseases. But time medicine is not the only factor in the process of disease occurrence and development, so it must not be blind in clinical treatment. Liu Zhu's theory of meridians is regarded as a master of TCM time medicine, which is a method that the ancients opened acupoints on time every day according to the daily rules of human qi, blood, viscera and meridians, combined with ten heavenly stems, twelve earthly branches, five elements of yin and yang and five Shu points (see the table below). For example: symptoms: headache, red eye, insomnia, spleen and stomach discomfort, indigestion: busy with work and entertainment, often go to bed at two or three in the middle of the night. Description: The running time of gallbladder meridian and liver meridian is 23:00-3:00. If you don't rest during that time, the liver will rise, leading to headache, congestion and discomfort. At the same time, due to the rise of liver qi, it affects the spleen and stomach, so I feel stomachache. Explained by yin and yang and five elements, the liver belongs to wood, the spleen belongs to soil, and the wood belongs to soil, so if the liver is uncomfortable, the stomach and spleen also have problems. Empirical Study on Chronological Nursing of Hypertension Patients with Yin Deficiency of Liver and Kidney Source: Gao Zhan, Feng Feng. Effect of health education combined with TCM time theory on extending nursing mode in hypertensive patients with liver-kidney yin deficiency [J]. Journal of Practical Cardiovascular Diseases, 2019,27 (3):107-165438. Hypertension belongs to dizziness, headache, stroke and liver wind in TCM theory, and its main pathological changes are in the liver. There is a cloud in Neijing: "All wind-induced dizziness belongs to the liver". The main physiological characteristics of the liver are active and ascending, but liver qi and liver yang are often excessive, and liver yin and liver blood are often insufficient. The common TCM syndrome type of hypertension patients is yin deficiency of liver and kidney, which is mostly caused by qi-blood damage and yin-yang imbalance caused by hunger, wine anger, overwork, blood loss and nocturnal emission. "Su Wen? As the saying goes, "the four seasons and five elements of the law are used to treat diseases", that is, the treatment should conform to nature and be treated according to the time. How to carry out research? Participants: 65,438+000 patients with hypertension of liver and kidney yin deficiency admitted to the Department of Cardiology, Affiliated Hospital of Shandong University of Traditional Chinese Medicine from June 438+July, 2065 were randomly divided into control group and intervention group, with 50 patients in each group. Inclusion and exclusion criteria: Inclusion criteria (1)35 ~ 70 years old; (2) 2) The Barthel index score is100; (3) No communication barriers; (4) Acceptable telephone, network contact and home visit; (5) Living in Jinan. Exclusion criteria (1) have serious dysfunction of heart, brain, lung, liver, kidney and other important organs and nerves; (2) Patients with mental illness and malignant tumor; (3) organ transplant recipients. Intervention method: Patients in the control group were given health education of TCM chronomedicine theory on the basis of routine antihypertensive treatment, while patients in the intervention group were combined with extended nursing mode on the basis of the control group. Health education based on TCM chronomedicine theory: Health education based on TCM chronomedicine theory was carried out according to hypertension symptoms and nursing points of liver and kidney yin deficiency type. The main contents are as follows: (1) Regularly monitor blood pressure: measure blood pressure at rest with arm electronic sphygmomanometer at 7:00 ~ 8:00 every day and record it in the Self-management Manual for Hypertensive Patients with Integrated Traditional Chinese and Western Medicine; (2) Take medicine regularly: within 24 hours, human blood pressure has two peaks (10:00 and 18:00) and two valleys (3:00 and 14:00), and patients are instructed to take medicine before the peaks to avoid the valley time; (3) Eat on time: inform patients to eat three meals on time every day, and instruct patients to eat more food such as Lycium barbarum, lean meat, red dates, tremella, hawthorn, chrysanthemum, auricularia auricula, sea cucumber, honey, mushrooms and lettuce. According to the dialectical principle of liver and kidney yin deficiency; (4) Life guidance: keep the home environment quiet, clean and soft, avoid strong light and noise, get up at 5:00~6:59 every day, and fall asleep at 2 1:00~22:59; (5) Time schedule: 19:00~2 1:00 is the best time for psychological care, which is conducive to patients' self-emotional adjustment, so medical staff will send music suitable for patients with hypertension due to liver and kidney yin deficiency through WeChat official account for 30 minutes during this time period; (6) Foot bath before going to bed: warm foot bath before going to bed, 1 time /d ... What is the research result? There was no significant difference in TCM symptom score between the two groups at discharge (P > 0.05). 0.05); At 4 weeks and 8 weeks after discharge, the TCM symptom score of the intervention group was lower than that of the control group, with statistical significance. The systolic blood pressure and diastolic blood pressure in the intervention group were lower than those in the control group at 4 weeks and 8 weeks after discharge, and the difference was statistically significant. Eight weeks after discharge, the CHPS score of the intervention group was lower than that of the control group, with statistical significance (P
