2 Introduction Splenic surgical diseases are rare.

3 Spleen abscess Spleen reticuloendothelial cells can phagocytose bacteria, so spleen abscess is rare in clinic. Occasionally, due to sepsis and endocarditis, embolus with bacteria enters the spleen, forming splenic abscess, which is often multiple. Spleen abscess caused by traumatic hematoma and infarction necrosis is mostly single. Because the pus cavity contains destroyed and dissolved spleen tissue, the pus is sticky.

Early symptoms of infection, followed by sepsis. Pain around the spleen, tenderness, abdominal muscle rigidity, pain intensifies with breathing, and sometimes radiates to the left shoulder. X-ray examination showed that the left diaphragm was elevated and its activity was limited. Splenectomy is necessary for huge splenic abscess that is ineffective in non-surgical treatment.

There are two kinds of splenic cysts: parasitic cysts and non-parasitic splenic cysts. Parasitic cysts are mostly splenic echinococcosis, which often coexist with liver and lung echinococcosis, especially in pastoral areas. Non-parasitic cysts can be divided into true cysts and pseudocysts. The true cyst wall is covered by epithelium, such as lymphocyst and dermoid cyst. The wall of pseudocyst is only composed of fibrous tissue, such as inflammation, arterial embolism or localized liquefaction after traumatic hematoma. Small cysts are mostly asymptomatic, and large cysts often have stomach compression. A swollen cystic mass can be palpated in the left upper abdomen, with a sense of fluctuation but no tenderness. Single-chamber cysts near the upper and lower poles are feasible for cyst resection or partial splenectomy; Patients with multilocular or giant tumors need splenectomy.

Spleen tumors are rare, and benign tumors are mainly hemangioma, lymphangioma and hamartoma. Lymphosarcoma and reticulocytoma are the main malignant tumors. Spleen metastasis can also be seen.

The diagnosis is not very difficult. The manifestations of splenic sarcoma are as follows: ① There is a mass in the left upper abdomen, which grows rapidly, or has a nodular feeling and often tenderness. ② The children have persistent or intermittent pain in the left upper abdomen, emaciation and anemia. Sometimes mild jaundice and low fever can be seen. (3) Barium examination showed that there was impression on the greater curvature of the gastric fundus, and the colon and splenic curvature moved to the lower right. Intravenous urography can show renal displacement. Splenectomy is the only treatment.

Wandering spleen's supporting ligaments around the spleen (spleen and stomach, spleen diaphragm, spleen and colon, spleen and kidney ligaments) relax due to congenital dysplasia, or stretch and lengthen due to spleen enlargement and abdominal muscle weakness, which makes the spleen lose its normal anatomical position and become wandering spleen. Its characteristic is that the spleen has great fluidity and can be relatively fixed in the later stage. Wandering spleen's spleen is bigger than normal, and torsion of spleen pedicle may occur. At the initial stage, the spleen is congested and swollen, then oozes and bleeds, and finally necrosis can occur. Simple splenic artery occlusion can cause spleen atrophy and fibrosis.

Clinical symptoms vary according to different lesions, and corresponding symptoms may appear when abdominal organs are compressed or pulled. Acute splenic pedicle torsion has severe abdominal pain and even shock. According to the situation, splenectomy, splenic autotransplantation or fixation were adopted.
