A 9-year-old girl in Guangzhou fell while practicing handstand, resulting in spinal cord injury and paraplegia.
Children should protect their spine when learning to dance, otherwise falling may really lead to spinal cord injury and paraplegia. Recently, a 9-year-old girl was admitted to the Department of Spinal Surgery of the Third Affiliated Hospital of Southern Medical University (referred to as the Third Affiliated Hospital of Southern Medical University), who fell down during dance practice, resulting in paralysis of both lower limbs and incontinence. Fortunately, after treatment, the girl's condition has gradually improved, and now she can walk in the fields.

The doctor suggested that children should dance step by step, don't rush for success, practice difficult movements, and at the same time make full warm-up preparations to avoid unnecessary injuries.

Practice handstand fall in dance class

Half a month ago, Xinxin (pseudonym), who lives in Haizhu District, accidentally fell down while practicing handstand in a dance class of a training institution. At that time, she felt weak and weak in her lower limbs, but after a short rest, she felt her legs regained strength, so she continued to practice, turn over, bend over and split. ...

But unexpectedly, things got worse again. Xinxin's mother recalled that when she went to pick up the child after the dance class, the child said that her legs were soft, but she didn't expect to be hurt so badly. Before long, Xinxin had difficulty walking, and she immediately sent her child to a nearby hospital.

Xinxin took a film examination in the emergency department of a nearby hospital and found no obvious fracture lesions. Later, the doctor advised her to transfer to another hospital, so Xinxin was sent to the Third Hospital of Southern Medical University.

Handstand, turning over and lower back movement lead to spinal cord traction injury and edema.

After coming to the Third Hospital of Southern Medical University, Wang Liang, the deputy chief physician of the First Department of Spinal Surgery, made a physical examination for Xinxin, and found that Xinxin's abdomen was swollen, her bladder was highly filled, urinary retention was accompanied by incontinence, and she was immediately indwelling catheter.

After improving the relevant examinations, it was found that Xinxin's X-ray showed normal spinal structure, and no abnormality was found. CT showed no abnormality in bones and the sequence was complete. MR examination of the whole spine showed edema of the spinal cord in T6- 10, and extensive patchy high signal shadow was seen in T2 phase.

After a series of examinations, the doctor found the cause of Xinxin's disease. It turned out that when dancing, handstand, rollover and lower back movements caused spinal cord traction injury and edema. Combined with the patient's medical history, physical examination and imaging examination, the initial diagnosis is: thoracic and spinal cord injury with paraplegia of both lower limbs.

Subsequently, neurology, neurosurgery, pediatrics, rehabilitation and other related departments made a comprehensive treatment plan for Xinxin. Administer nourishing nerve to relieve nerve edema; Rehabilitation exercise mainly uses hyperbaric oxygen, acupuncture, electrical stimulation and other methods to promote nerve recovery, and passive physical exercise to prevent muscle atrophy; Rehabilitation nursing is mainly to prevent long-term bedridden complications such as venous thrombosis, bedsore, bedsore, lung and urinary tract infection.

After medical treatment, nursing and rehabilitation, Xinxin's symptoms of both lower limbs paralysis gradually improved.

Spinal cord injury in children without fracture and dislocation: common practice in children dance

According to Wang Liang, children's spinal cord injury without fracture and dislocation is a common type of traumatic spinal cord injury in children (accounting for 19%-34%), which is clinically called children's spinal cord injury without fracture and dislocation (SCIWORA). In China, this kind of injury is more and more common in children dance's handstand and lower back movements, and the injured segments are mostly chest segments.

"In recent years, with more and more children participating in dance classes to practice dancing, this kind of injury has become more and more common, especially for girls." In clinical work, SCIWORA is caused by car accidents, falling from a height, etc. It is easy to diagnose, but SCIWORA, which is caused by non-serious violence such as dancing and bending over, is often easily overlooked in the early stage and delays treatment.

Injury mechanism: external force causes the vertebral body to reset itself after instantaneous displacement.

Thoracic spinal cord injury is common in children and adolescents without fracture and dislocation, which can be caused by jumping (trampoline), bending, handstand, falling, hitting the waist and buttocks or falling to the ground while practicing acrobatics.

The mechanism of this injury is that the external force makes the vertebral body move instantly and then reset itself, especially for children under 8 years old. Slight external force can make the vertebral body move to a greater extent, and the degree of injury is higher than that of adults.

Common symptoms: weakness of lower limbs, but symptoms may not appear immediately.

The common symptoms of spinal cord injury are paraplegia, paresthesia, bladder and rectum dysfunction (urinary retention, urinary leakage, urinary incontinence, etc.). ). However, the onset time of such symptoms will be different: some children will have symptoms immediately after the injury, and some children will have delayed symptoms (20%-25%) after the injury, ranging from 10 minutes to 4 days after the injury.

Wang Liang reminded that parents should attach great importance to the above symptoms after the child is injured. Be sure to let the child rest on the spot and don't move the child at will. For children who have recovered from minor injuries, they must stop doing difficult dance movements and go to the hospital in time. For children with severe complete paralysis, parents should not "hold the princess" on their chest during medical treatment, which will aggravate spinal cord injury. Axis rotation method is the correct way to transfer flat cars.

"In the process of dancing, children who have no dance foundation should not rush for success, practice some difficult movements, but follow the principle of step by step. In addition, you should be fully prepared for warm-up before practicing dance to avoid unnecessary damage. " Wang Liang suggested.
