Why do patients with cervical spondylosis often choose Chinese medicine for treatment?
The fourth in a series of reports on "TCM talking about disease prevention and health preservation"

"Cervical spondylosis is a common disease with a high incidence. Many patients with cervical spondylosis are willing to choose Chinese medicine for diagnosis and treatment. " When talking about the treatment of cervical spondylosis, Deputy Chief Physician Liu of the Department of Traditional Chinese Medicine of Meizhou People's Hospital said, "The reason is that Western medicine mainly uses traction and small needle knife loosening to treat cervical spondylosis. If it is serious, surgery is usually taken. Compared with other orthopedics, the risk of cervical spondylosis surgery may be higher, because the spinal cord belongs to the central nervous system of human body. If the operation is wrong during the operation, it may cause local spinal nerve injury, resulting in numbness of limbs, local pain and limited activity. " According to him, over the years, he has treated nearly 1,000 cases of cervical spondylosis with Chinese medicine, mainly divided into the following five types.

Neck type

The main manifestations are: pain in the head and neck, shoulders and back, unfavorable lateral rotation, difficulty in pitching and rotating, neck fatigue, red tongue, thin and white fur and thready pulse.

Treatment principle: relax tendons and activate collaterals, dispel wind and relieve pain.

Treatment: ① Acupuncture: acupuncture Jiaji, Ashi, stiff neck, Fengchi and Hegu points by straight needling or oblique needling and diarrhea, warm moxibustion or TDP irradiation for 30 minutes, needle walking every 5 minutes when the needle is kept, cupping after needle drawing, and then cupping for 5 minutes. ② Massage: Apply the neck and shoulder muscles with gentle one-finger Zen massage and rolling method for about 10 minute; Then use the rotating lifting method, and finally rub the neck and shoulders, lift the shoulders well, and beat the back muscles of the neck and shoulders for about 15-20 minutes. ③ Traditional Chinese medicines: Radix Puerariae, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Radix Paeoniae Alba, Notopterygii Rhizoma, Radix Saposhnikoviae, Radix Gentianae Macrophyllae, Ramulus Cinnamomi, and Radix Glycyrrhizae, which are treated according to the symptoms, and decocted in water once a day, twice orally, and the residue is hot applied to the neck and shoulders.

Treatment results: Cervical spondylosis is treated once a day, and 5 times is a course of treatment. Generally, it can be cured within one course of treatment, and the symptoms can disappear after a short course of treatment of 1-2 times.

Vertebral artery type

The main manifestations: headache, neck pain, dizziness, and related to body position, even sudden collapse, pale tongue, white and greasy fur, and smooth pulse.

Treatment principle: promoting blood circulation and resolving phlegm, relaxing tendons and activating collaterals.

Treatment: ① Acupuncture: Acupuncture at Fengchi, Tang Yin, Temple, Neiguan and Jiaji points in the neck, with the method of flat reinforcing and purging for 30 minutes, and warm moxibustion or TDP irradiation for 45 minutes in the neck. ② Massage: Massage the neck with one finger Zen, and press the acupoints such as Fengchi, Taiyang, Sishencong Tinggong and Tingtang in a rolling way. According to the tenderness and deviation of spinous process, the fixed-point rotation reduction method and pre-tension reduction method were used, and finally the back was patted with the head, neck and shoulders for 20 minutes each time. ③ Chinese medicines: Poria, Atractylodes rhizome, Rhizoma Fagopyri Preparata, Radix Salviae Miltiorrhizae, Arisaema cum bile, Radix Puerariae, Radix Paeoniae Alba, Radix Clematidis, Ramulus Cinnamomi, Radix Angelicae Dahuricae, Radix Bupleuri, Rhizoma Gastrodiae, Flos Chrysanthemi, etc. Will increase or decrease with the symptoms.

Treatment results: The incidence rate is about 15%. This method is once a day, and 5 times is 1 course of treatment. For cervical spondylosis of vertebral artery type caused by simply compressing the cervical vertebral artery, the fixed-point rotation reduction method can be effective at one time, and the symptoms can be controlled in one course of treatment. Combined with vertebral arteriosclerosis and other factors, the comprehensive treatment effect is better. Judging from the clinical treatment, the total effective rate is above 85%.

Sympathetic nerve type

The main manifestations are: dizziness, dizziness, tinnitus, numbness of hands, tachycardia and other autonomic nervous dysfunction, which is related to the movement and posture of cervical spine. The tongue is pale, the coating is thin and white, and the pulse is thin.

Treatment principle: relax muscles and collaterals, and promote qi and yang.

Treatment: ① Acupuncture: Take Fengchi point, Fengfu point, Baihe point, Neiguan point, Quchi point, Zusanli point and Sanyinjiao point for acupuncture, mainly to tonify deficiency and stop diarrhea, and acupuncture on the neck for 30 minutes. ② Massage: One finger Zen massage, neck rolling. According to symptoms: point, press and rub common acupoints on the head and face; Pay attention to the symptoms of limbs and the operation of limbs; Cardiovascular symptoms are the main symptoms, according to Xinshu, Shenshu, Ganshu, Mingmen, Neiwaiguan and Hegu. ③ Drugs: Radix Puerariae, Radix Salviae Miltiorrhizae, Ramulus Cinnamomi, Rhizoma Chuanxiong, Scorpio, Scolopendra, Lumbricus, Semen Ziziphi Spinosae preparata, Fructus Schisandrae Chinensis, Poria, Caulis Polygoni Multiflori, Radix Angelicae Sinensis, Radix Glycyrrhizae, etc. And add and subtract symptoms.

Treatment results: Typical sympathetic cervical spondylosis accounts for about 5%. Because of its complicated symptoms, it is often easy to be misdiagnosed and mistreated. This treatment method is 1 time per day, and 1 time is1course of treatment. Because simplicity is rare, it is often mixed with other types, so the effect is not good.

Spinal cord type

The main manifestations are: stiff neck pain, heavy limbs, numbness, chills, pain in both lower limbs, unsteady walking, trembling and weakness like "feeling of stepping on cotton", severe paralysis of lower limbs, uncontrolled defecation, pale or red tongue, less fur and weak pulse.

Treatment principle: regulating qi and blood, dredging meridians and strengthening bones and muscles.

Treatment: ① Acupuncture: Acupuncture Jiaji, Shenshu, Mingmen, Weizhong, Zusanli, Shousanli, Neiwaiguan, Hegu and other points, mainly reinforcing methods, applying warm moxibustion or TDP irradiation to the neck, and keeping the needle for 30 minutes. ② Massage: Massage the neck and shoulders with one finger for about 65,438+00 minutes, and massage the affected limbs with points, presses, rubs and rubs. In supine position, the patient massages bilateral Fengchi points, stirs the neck muscles, and pulls the neck. Use with caution. ③ Traditional Chinese medicines: Radix Rehmanniae Preparata, Rhizoma Dioscoreae, Fructus Corni, Fructus Lycii, Radix Angelicae Sinensis, Poria, Radix Dipsaci, Achyranthis Radix, Radix Astragali, Carapax et Plastrum Testudinis, Colla Cervi, Herba Epimedii, Radix Puerariae, Radix Salviae Miltiorrhizae, Cortex Phellodendri, Rhizoma Anemarrhenae, Herba Cynomorii, Atractylodis Rhizoma, Fructus Chaenomelis, Pericarpium Citri Tangerinae, etc. And add and subtract symptoms. One dose a day, decocted in water twice orally, and the residue fumigated the neck of the affected limb.

Treatment results: the incidence rate is about 10%, and this treatment method is only suitable for patients with short course of disease and mild symptoms; Once a day, 20 times as a course of treatment, generally 1-3 courses of treatment. Surgical treatment should be considered for patients with severe symptoms or advanced and aggravated symptoms.

Nerve root type

The main manifestations are: head and neck pain, occipital pain, numbness and pain in shoulder and back, one or two upper limbs and fingers, head and neck activity or cough and sneezing aggravating symptoms, with obvious radiation pain and numbness. The tongue is pale, the coating is white, and the pulse is thin.

Treatment principle: promoting blood circulation, dredging collaterals, relieving spasm and relieving pain.

Treatment: ① Acupuncture: Take Jiaji point in the affected neck, Fengchi point in the affected shoulder, Quchi point, Waiguan point, Hegu point and Houxi point, and use the method of flat reinforcing and purging, or use the method of reinforcing and purging in the short course, and keep the needle for 30 minutes in the long course, and use local warm moxibustion or TDP irradiation or electroacupuncture treatment. ② Massage: After 65,438+0.5-20 minutes of cervical traction, massage and roll with one finger at the neck and shoulder to relax the neck muscles, then massage the affected limb's shoulder, Shousanli, Quchi, Neiwaiguan, Hegu and other acupoints, and then adopt fixed-point traction according to the compression of nerve roots. Finally, pull the neck muscles, lift the neck and shoulders, and rub the affected limb. ③ Traditional Chinese medicines: Radix Astragali, Radix Angelicae Sinensis, Rhizoma Chuanxiong, Ramulus Cinnamomi, Scorpio, Scolopendra, Lumbricus, Rhizoma Corydalis, Radix Paeoniae Alba, Radix Rehmanniae Preparata, Radix Clematidis, Radix Puerariae, Radix Salviae Miltiorrhizae, Herba Taxilli, Notopterygii Rhizoma, Radix Angelicae Pubescentis, etc. Will increase or decrease with the symptoms. Decoct one dose of water twice a day.

Results: The incidence of cervical spondylotic radiculopathy was high, accounting for about 60%. Treatment is generally 1-2 times a day, and 10 times is a course of treatment. 1-3 courses of treatment, the effect is stable, but recurrence should be avoided. In addition to paying attention to the neck posture, it is also necessary to prevent the neck from catching cold and strengthen the neck function exercise. The comprehensive treatment has obvious effect, and the effective rate is over 90%.

Exercise relieves cervical spondylosis

"A considerable number of people suffer from cervical spondylosis because of bad living habits. Because with the large-scale use and popularization of computers and mobile phones, there are many low-headed people. The best treatment is to change bad habits, bow your head as little as possible, don't work at your desk for a long time, and don't lie in bed watching TV and playing mobile phones. Only by changing bad living habits can cervical spondylosis be fundamentally treated, "Liu reminded.

He suggested that patients with cervical spondylosis choose appropriate exercise, which is not only a treatment method, but also an extremely important means to consolidate the curative effect. Because the cervical spine is the largest part of the whole spine, there are few opportunities to maximize activities in daily life. However, the aging and degenerative changes of cervical spine affect its physiological function and cause a series of clinical symptoms. Through exercise, the physiological function of the patient's neck can be enhanced, the symptoms can be eliminated and the curative effect can be consolidated. But exercise must be targeted. If the wrong exercise method is adopted, it will not only relieve the illness, but also aggravate the pain, destroy the normal cervical facet joint structure and cause serious consequences. He said that the best exercise method for cervical spondylosis is the functional exercise of neck and shoulder muscles. Common clinical exercises include:

1. Bend forward and stretch back: bend the neck forward as far as possible, stay for 3 seconds, then lean back, stay for 3 seconds, and make two 8 beats;

2. Look around: turn your head 90 degrees to the left, stay for 3 seconds, then turn right, stay for 3 seconds, and make two 8 beats;

3. Shrugging exercise: shrug your shoulders, stay for 3 seconds, and do two 8 beats;

4. Chest expansion exercise: chest expansion tightens the neck and back muscles, stays for 3 seconds, and makes two 8 beats.
