Under the guidance of the major policies of the Party and the country, Mafang Hospital, with the aim of "serving patients wholeheartedly", explored the basic medical and health service model in rural areas (communities), "seeking novelty, seeking truth and seeking efficiency", considered the new service model from the people's point of view, carefully formulated and adhered to a well-thought-out plan, and embarked on the exploration road of caring for patients in the community, which was well received by the people. Health link is open 24 hours a day.

In order to meet patients' health consultation needs, so that patients can get timely, meticulous and scientific medication guidance and medical and health policy information, President Qu Yongcheng of Mafang Hospital, after consulting with clinicians, arranged to set up a 24-hour "Health Link" consultation hotline, and approved the printing of the Doctor-Patient Contact Card of Mafang Hospital, on which the telephone number of director Zhang Yarong, a health consultant and clinical attending physician of Mafang Hospital, was prominently printed. In the lecture hall of "Patient Health Education", the elaborate design of small cards with telephone numbers was displayed, which made it more convenient for villagers to consult prevention, health care and medication knowledge at the first time, so that villagers could get the intimate service of "family doctors" and the communication between doctors and patients was "faster, more timely and more convenient". Since the publication of Doctor-Patient Contact Card in Mafang Hospital, the health consultation hotline has been connected to 1000 times, which has answered thousands of villagers' health consultations and provided medication guidance for hundreds of individual patients.

Chapter II: Summary of the Annual Work of the Community Health Service Center Under the care and leadership of the District Party Committee, the District Government and the District Health Bureau, with the support and help of the Party Working Committee of Shuanggang Street, the office and the departments of disease control, health care and supervision in the district, the leading group of the center took the opportunity of the in-depth development of the activity of striving for Excellence, conscientiously implemented the spirit of the series of documents on the reform of the primary medical and health system, and strengthened management with the emphasis on the gradual equalization of basic public health services. Through the joint efforts of the medical staff in the center, the work of the center has been greatly improved, and the tasks assigned by the superiors have been successfully completed, which has effectively promoted the healthy development of community health services. The work is now reported as follows:

I. Basic information

(1) The employment center has a business area of nearly square meters and serves about 10000 people. Since 20 10, the center has determined specific posts, defined post grades, responsibilities and tasks, work standards and qualifications, and organized personnel to compete for posts, employ according to posts and sign employment contracts. The center has 40 professional technicians and 40 posts approved by the District People's Social Security Bureau. At present, there are 6 middle-level professional titles, 0/5 medical practitioners1person (including general practitioners1person, and 2 under training) and general nurses1person, and the general service has been established.

There are 10 teams (6 centers and 4 service stations).

(2) The basic medical facilities of the Hardware Construction Center have been greatly improved, with complete facilities, comfortable environment and a brand-new overall appearance, which are visited and studied by delegations from various provinces and cities. Newly purchased 1 imported general diagnosis and treatment instruments, newly added 1 urine sediment analyzer, newly added general outpatient service (1-5), expanded outpatient infusion room, newly added observation room and newly built 30 beds. However, the center develops the characteristics of integrated traditional Chinese and western medicine, and the existing equipment can not meet the needs of the development of traditional Chinese medicine.

(III) Basic medical care and income and expenditure From 20101kloc-0/,all drugs in our center are sold at zero difference rate. At the same time, the standardized management of hospital infection was strengthened, which was well received by the inspection team in Hefei community health assessment.

Continue to promote public hospitals to support community health work. Our center signed a counterpart support agreement with Hefei First People's Hospital. Experts from the First Hospital of Hefei sit in the clinic four and a half days a week for consultation and guidance on rational drug use. Experts * * * counted more than 200 times throughout the year. At the beginning of this year, we opened a rehabilitation ward by contacting the appropriate technology of Chinese medicine. At present, 122 people have been admitted. People in the community can enjoy expert outpatient service at their doorsteps, and they don't have to wait in line in big hospitals, which is very popular among community residents.

This year, the center also undertook the tasks of establishing the National Model Community Health Service Center, the Training Base of China Community Health Association, the HIV antibody detection point in Hefei, and the standardized pharmacy in Hefei.

Implementation of zero-difference rate of service stations: On April 25th and May 24th, our center supervised and guided the "zero-difference rate" of six service stations in the jurisdiction. From the inspection, the overall situation is good, but there are still the following problems:

1. The essential drugs prescribed by all service stations are not separately prescribed and kept. 2. Except Xiaogang service station, the drug subsidiary ledger of other service stations has not been established or is not perfect. 3. The price of provincial supplementary drugs purchased by Xinghua Service Station is higher than the provincial bidding price, the ratio of the total amount of self-selected drugs to the total amount of drug purchases is ≥ 104, and the addition rate of self-selected drugs in Lvdu Garden Service Station is ≥ 13. The outpatient log registration is incomplete, some patients are not registered, and the prescription Chinese medicine fee is not priced separately from the material fee. 5. The purchase documents of Huaixi and Xinghua community service stations have not been seen yet, so whether the drug price announcement is the same as the retail price needs further confirmation.

6. The drugs in each service station were not counted on time as required.

Our center will strengthen the supervision of service stations according to the requirements, and urge service stations to standardize the implementation of the "zero difference rate" of drugs.

Third, there are difficulties 1. Due to the large population under the jurisdiction of the center and the multi-functional staff, the filing rate of health records in the center, the management rate of chronic diseases and the elderly need to be improved.

2. Children's health care is sinking, the task is getting heavier and heavier, and the staffing can't meet the work requirements.

3. There is still a big gap from the assessment index in the work of establishing "National Model Community Health Service Center" in this center, and all the work needs to be further improved.

Fourth, the next step is to plan (1) basic medical services 1, strengthen publicity and further improve basic medical services. Make full use of the advantages of western medicine counterpart support in Hefei First People's Hospital and Chinese medicine counterpart support in the hospital of integrated traditional Chinese and western medicine to carry out large-scale publicity and free clinic activities in residential areas. Increase the publicity of the center's community health service, essential drug list, medical insurance policy and preferential policies, improve the social awareness of our center and increase the trust of community residents.

2. Improve the functions of hospital management information system and software query system, improve the accuracy of all kinds of information statistics in the center, and provide a basis for assessing the workload of medical staff.

3. By establishing a counterpart support mechanism for traditional Chinese medicine, opening a rehabilitation ward for traditional Chinese medicine, introducing technology, talents and management experience from hospitals of integrated traditional Chinese and western medicine, strengthening the capacity building of community traditional Chinese medicine services, and giving full play to the advantages and functions of traditional Chinese medicine in community health services. At the same time, taking the opportunity of standardization of traditional Chinese medicine, we should increase the appropriate technology of traditional Chinese medicine and measures to benefit the people, add Chinese medicine elements, highlight the appropriate technical characteristics of traditional Chinese medicine, and face the community residents.

(2) Basic public health services 1, improving the filing rate and updating rate of health records, chronic disease management rate and standardization rate.

2. Improve the normalization mechanism of free physical examination for the elderly and chronic diseases. Physical examination, filing and follow-up should be closely linked, and the coordination mechanism of each team and physical examination department should be improved. Flexible use of performance appraisal scheme, regular quantification, public performance pay grade, improve the enthusiasm and urgency of medical staff.

3. Strengthen the management of general practice service team, improve the coordination ability of general practice team leaders, and gradually establish and improve the working mechanism of "family doctor" contacts.

4. Taking the opportunity of striving for "National Demonstration Community Health Service Center", give full play to the typical radiation function, and activate the overall improvement of community health service by strengthening the construction of demonstration models. We will equalize nine basic public health services, including health records, health education and chronic disease management, so that community residents can feel the benefits of the reform of primary health care system.

5. Improve the comprehensive management mechanism of service stations in the jurisdiction, and strengthen the training and supervision of service stations.

Chapter III: Summary of Community Hospital Work Five years ago, when people around me learned that I was a preventive medicine major, their first reaction was: Oh, I got a vaccine. At that time, the concept of "prevention" was narrow in everyone's mind, and the awareness of prevention and health care was not as strong as it is now. Clinical and prevention, one in front of the stage, one behind the scenes, one facing patients and one facing the masses. Clinicians in front of the stage treat patients and save lives, and behind-the-scenes preventive workers escort them, both of which are indispensable. There is an old saying in China that preventive care is more appropriate. When I came to the community at the end of July, my first contact was the survey of all kinds of people, large and small: health knowledge questionnaire survey, colorectal cancer screening, the fifth national health service survey, and monitoring of chronic diseases and risk factors. In this process, I deeply realized that the "bridge" function of neighborhood committees is irreplaceable, which is related to residents' understanding, trust and cooperation in our investigation work. Before the investigation, we need to publicize the purpose and importance of our investigation through the neighborhood Committee. In addition, investigators should also take the pre-survey training seriously, master the main points of the questionnaire survey, and avoid rework in the later stage, resulting in a waste of time and manpower. From 1 1 to now, I began to participate in the planned immunization work, mainly vaccination registration and appointment. In just two months, I have a deeper understanding of prevention work. Different from clinic, prevention is not an immediate job, but vaccination is the most effective one. If the previous prevention work was very big and shapeless for me, now I can get a glimpse of it through planned immunization, which makes me feel down to earth. At the time of registration, the parents who have the most contact are the children's parents, with a long queue and hundreds of people. Good order maintenance is the fundamental guarantee. When parents disagree with us, on the one hand, we should communicate well in time, on the other hand, the intervention of a third party can often alleviate the problem. Doing every simple thing well is not simple, and doing every ordinary thing well is extraordinary. Details determine success or failure. Community work is meticulous in many aspects and needs our serious and responsible treatment. I believe that in the near future, through the continuous efforts of our public health workers, residents' disease prevention and health promotion will make great progress!

Chapter four: Working experience of community hospitals. Community health service centers are health institutions that focus on prevention and combine prevention with treatment. Shanghai is in a leading position in this respect, and because of this, I chose to work in Shanghai. I have been working for nearly half a year now. Through continuous work, I have gradually grown from a student to a community health worker. In this process, I have a lot of thoughts and feelings about my work.

Practice is the only criterion for testing theory. Just entering a new post, I learned the theory from school, and I don't know too much, so I'm afraid. As a newcomer, work is not only about learning, but also about daring to do it. Don't stand behind the teacher and watch for fear of making mistakes. Only through continuous practice can we find our own shortcomings, learn more purposefully and constantly improve our professional level. Of course, daring to do it does not mean blindly getting started. In this process, we need to read more books, ask more questions, remember more, practice in learning and learn in practice.

Business learning is important, but daily learning can't be ignored. Knowledge in modern society is updated rapidly. Only by constantly learning, updating our knowledge base and adapting to the changes of time and environment can we adapt to the increasingly fierce competition and be competent for our own work, otherwise we will eventually be eliminated by reality. At the same time, we also need to constantly learn and improve our own thinking, so that our thinking is always consistent with the party and organizations, so as to ensure that we do not deviate from the correct track in practical work.

Working in the community has made me deeply realize that as a grassroots health worker, I shoulder the heavy responsibility and due responsibility. In practical work, everything needs to be treated with caution. In the prevention department, the most common contact is all kinds of data and forms. Maybe a wrong number will ruin a table, maybe a wrong word will lead to a duplicate name. So before I do anything, I will remind myself to be serious. Being responsible for things is also responsible for myself. In the work, we should adjust our position according to the division of labor, not offside, not leaving our posts, strictly observe our duties and complete our work well. Only by doing a good job of self-positioning, clarifying responsibilities, uniting with comrades, being sincere, down-to-earth, being loyal to their duties, being diligent and taking one step at a time can we do our job well and do our job well for primary health workers.

In the new year, I will study more, consult more, improve my own shortcomings, take every step in a down-to-earth manner, and strive to achieve good results in my work.
