To apply for health food registration, the following materials shall be submitted:

1, health food registration application form, and the applicant's legal responsibility commitment letter for the authenticity of the application materials;

Product R&D report, including R&D personnel, R&D time, R&D process, verification data above pilot scale, demonstration report, relevant scientific basis for safety, health care function and quality controllability of raw materials and products outside the catalogue, and product technical requirements comprehensively determined according to R&D results, etc. ;

4, product formula materials, including the name and dosage of raw materials, production technology, quality standards, if necessary, should also provide the basis for the use of raw materials, use parts, inspection certificate, variety identification report, etc. ;

5, product production process information, including production process flow chart and description, key process control points and description;

6, safety and health function evaluation materials, including raw materials and products outside the directory safety and health function test evaluation materials, human consumption evaluation materials; Test reports on functional ingredients or symbolic ingredients, hygiene, stability, strain identification and strain toxicity, as well as test reports on stimulants and prohibited drugs;

7, direct contact with health food packaging materials, names, related standards, etc. ;

8. Labels and descriptions of sample products; Retrieve the information that the generic name in the product name is inconsistent with the registered drug name;

9, 3 minimum sales packaging samples;

10, other materials related to product registration review.
