Plain scan and CT signs show that the left lobe of the liver is obviously enlarged and shows low density, and irregular patchy low density area can also be seen in the right lobe of the liver, with unclear boundary and widened hilar area (Figure 5-2-4A and B). Contrast-enhanced scanning showed uneven enhancement of the liver, and many low-density areas of different sizes could be seen. The portal vein of the main liver and its left and right branches are not enhanced, and irregular low-density areas can be seen (Figure 5-2-4C). There are many tiny obviously enhanced collateral vessels around it (Figure 5-2-4D).
CT diagnosis ① diffuse hepatocellular carcinoma complicated with hepatic portal vein tumor thrombus formation; ② Hepatic portal vein thrombosis.
Finally, it was diagnosed as diffuse liver cancer with tumor thrombus formation in hepatic portal vein.
The invasion of hepatic portal vein system and the formation of tumor thrombus are the most important formation of intrahepatic diffusion, and also the main factors affecting the prognosis and interventional therapy of patients. The probability of invasion of hepatic portal vein and formation of tumor thrombus is related to the type of dirty love, the size of lesion and the length of course of disease. More than 90% of diffuse hepatocellular carcinoma (HCC) is complicated with hepatic portal vein tumor thrombus formation, and more than 70% of giant nodular HCC has invasion of hepatic portal vein and tumor thrombus formation, while nodular HCC is rare, only about 3%. Portal vein invasion of hepatocellular carcinoma is more common in branch vessels, but it cannot invade the main body. The formation of tumor thrombus in different parts of hepatic portal vein is related to the location of tumor, and the right page tumor often involves the left branch and trunk of hepatic portal vein.
