Darwin's No.7 health notice includes medical treatment behavior and insurance coverage, insurance coverage, diseases or symptoms, physical abnormality, abnormal examination, children under two years old (inclusive), female information, occupational status and normal insurance coverage. Details are as follows:

1. Medical treatment behavior and insurance situation: whether the insured has been hospitalized continuously (7 days or more) due to illness or abnormal health, or has received surgical treatment or continuous medication for more than 20 days, or has been recommended to be hospitalized or operated by a medical examiner or doctor in the past 2 years;

2. Insurance coverage: When the insured applies for insurance coverage or compound life insurance, critical illness insurance or cancer insurance policies from other insurance companies, has the insurance coverage ever been refused, postponed, increased or excluded? Have you ever made any claim for critical illness insurance or cancer insurance? Excluding the products insured this time, whether the cumulative coverage of critical illness insurance and cancer prevention insurance of other insurance companies exceeds 6.5438+0 million;

3. The insured currently suffers from or has suffered from the following diseases or symptoms:

(1) Tumor-related diseases: malignant tumor (including leukemia and Hodgkin's disease), borderline tumor, low-grade malignant tumor, potential low-grade malignant tumor, carcinoma in situ, atypical hyperplasia, intestinal metaplasia, brain tumor, thyroid nodule, breast nodule and lung nodule;

(2) circulatory and respiratory diseases: hypertension (systolic blood pressure > 140 mmHg or diastolic blood pressure > > 90mmHg), coronary heart disease, angina pectoris, myocardial infarction, arrhythmia, cardiac insufficiency, heart valve disease, pulmonary hypertension, cor pulmonale, cardiomyopathy, pericardial disease, ventricular aneurysm, endocarditis, aortic disease and congenital heart disease; Respiratory failure, chronic bronchitis, emphysema, chronic obstructive pulmonary disease, asthma, tuberculosis, pulmonary embolism, pneumoconiosis, interstitial lung disease and pulmonary fibrosis;

(3) Nervous system and mental diseases: stroke, cerebral ischemia, cerebral hemorrhage, subarachnoid hemorrhage, intracranial vascular malformation, intracranial hemangioma, encephalitis, meningitis, hydrocephalus, brain injury, poliomyelitis, multiple sclerosis, muscular dystrophy, myasthenia, motor neuron disease, paralysis, cerebral palsy, epilepsy, Parkinson's disease, dementia, schizophrenia, personality disorder and depression.

(4) Endocrine diseases and immune system diseases: diabetes, gout, hyperthyroidism, hypothyroidism, hyperparathyroidism, adrenal cortex hypofunction, aldosteronism, rheumatoid arthritis, rheumatic fever, Sjogren's syndrome, mixed connective tissue disease, systemic lupus erythematosus, systemic sclerosis (scleroderma) and ankylosing spondylitis;

(5) Diseases of blood and lymphatic system: anemia, leukemia, hemophilia, purpura, splenomegaly, myeloproliferative diseases, coagulation dysfunction, gangrene and elephantiasis;

(6) Digestive system diseases: chronic hepatitis B, hepatitis C, alcoholic hepatitis, liver cirrhosis, liver decompensation, polycystic liver disease, hepatolenticular degeneration, chronic atrophic gastritis, Crohn's disease, ulcerative colitis, intestinal obstruction and chronic pancreatitis;

(7) Urinary system diseases: chronic nephritis, glomerular diseases, nephrotic syndrome, pyelonephritis, renal insufficiency, renal failure and uremia;

(8) Diseases of skin, limbs and five senses: deep second or third degree burns, spina bifida, osteodystrophy, necrotizing fasciitis, severe osteoporosis, glaucoma, high myopia (1000 degree or above), optic nerve or retinopathy, blindness, hearing loss or deafness, and language disorder;

(9) Others: organ transplantation or hematopoietic stem cell transplantation, disability, coma, vegetative state, mental retardation, congenital diseases, hereditary diseases, chronic poisoning, sexually transmitted diseases, AIDS or HIV carriers, and the history of drug use or prohibited drugs;

4. Have the following physical abnormalities in the last 6 months: repeated headache, hemoptysis, bloody or black stool, hematuria, dysphagia, chest pain, edema, proteinuria, repeated subcutaneous bleeding, purpura, unexplained persistent fever, swollen lymph nodes, and emaciation (weight loss of more than 5 kg due to non-fitness reasons);

5. Are there any of the following conditions in the last two years: one or more of the following tests are abnormal, and doctors suggest follow-up, reexamination or further examination: tumor markers, pathological biopsy, cytology, cardiac ultrasound, CT, MRI, PET-CT, fundus examination and endoscopy;

6. Children under two years old (inclusive) are informed: Is the weight below 2.5 kg at birth, and is there a history of premature delivery, dystocia, asphyxia, intracranial hemorrhage and rescue at birth? Or have developmental retardation, convulsion, convulsion, cerebral palsy, deformity or defect;

Normal insurance can still be applied in the following situations: natural childbirth, induced abortion, colds, rhinitis, sinusitis, acute bronchitis, acute gastroenteritis, cured cholecystitis, cholecystolithiasis, appendicitis, lipoma, hemorrhoids, circumcision, cured limb fractures and skin trauma.
