(two) the formula, production technology and quality standards of health food;

(a) toxicological safety evaluation report;

(five) the list of functional ingredients of health food, as well as the qualitative and/or quantitative inspection methods and stability test reports of functional ingredients. Because the functional ingredients cannot be defined under the existing technical terms, the list of main raw materials related to health care functions in food must be submitted;

(8) Relevant information at home and abroad;

(nine) other materials that should be submitted according to the relevant provisions or product characteristics.

The holder of the health food approval certificate can use the certificate to transfer technology or cooperate with other parties in production. At the time of transfer, the transferee shall jointly apply to the Ministry of Health for a copy of the health food approval certificate. When applying, you should hold a health food approval certificate and provide a valid technology transfer contract. A copy of the health food approval certificate is sent to the transferee, and the transferee has no right to transfer the technology.
