[Case Report] Detailed Analysis of Inguinal Pain
Yesterday, I met a case of groin pain+weakness of lower limbs for nearly two years.

To tell the truth, inguinal pain is difficult to treat as long as it lasts for more than half a year, especially for 24 hours. Come to me when it's serious, and it will work faster. When the chronic phase is flat, the patient has psychological shadow and the recovery will be very long.

The following is the analysis report I wrote to the other party.

"Inguinal pain" analysis: There are only two reasons, either local tissue problems or distant influences.

Local problems, depending on what organizations exist in the local area, everyone is suspected; The problem in the distance is mainly the influence of nerve and fascia chain (equivalent to meridian/tendon).

Look at the local organizations first:

1. Skin and subcutaneous tissue

2. Lymph nodes

3. Muscles include: iliopsoas, rectus femoris, sartorius, pubis (adductor), medial oblique, lateral oblique, transverse abdominis and rectus abdominis.

4. Bone joints include ilium, pubic bone, femur, sacroiliac joint, hip joint and pubic symphysis.

5. Ligaments include: groin ligament, shame thigh ligament, iliofemoral ligament, round ligament of uterus (female) and spermatic cord (male).

6. The blood vessels are femoral artery, femoral vein and spermatic vein (male).

7. Nerve: Most of the lumbar plexus nerve passes through the groin.

8. Internal organs include: urinary tract, appreciation, and some organs in the lower digestive tract.

9. Space-occupying tissue: cyst, tumor, hernia, etc.

Look at the influence of distance:

1. Nerve and lumbar plexus include:

These nerves are related to pain and discomfort in the groin area. Among them, iliohypogastric nerve, ilioinguinal nerve and genitourinary femoral nerve directly control the skin sensation in inguinal region, while femoral nerve and obturator nerve directly control the sensation of hip joint (deep inguinal sac).

Baidu, you can find:

2. Fascia chain, which mainly includes anterior surface chain, anterior deep chain, spiral chain and anterior functional chain, connects the groin area and the muscle fascia of the body.

3. Meridian/Meridian. The liver, gallbladder, spleen, stomach and kidney in the twelve meridians all pass through the groin, and some of the eight strange meridians also pass through.

To sum up, any of the organizations mentioned above may have groin pain and discomfort.

So which one do you belong to?

After evaluation, I think it is mainly three factors:

1. Bone joint: the left iliac bone is displaced backward and downward. This can be confirmed by X-ray and palpation.

2. Muscles: iliopsoas and rectus femoris contracture. Thomas test is positive, stretching is painful.

3. Nerve: The ilioinguinal nerve and iliohypogastric nerve can be compressed, and the soft tissue near the articular process and transverse process of the first and second lumbar vertebrae on the left is tender.

therapeutic regimen

A loose: relax the tense muscles and ligaments, and it won't hurt. After the muscles are relaxed, the bones can be straightened and the nerves will be protected from muscular oppression.

Erzheng: pelvis dislocation is correct and force line is incorrect.

Three exercises: first strengthen the weak muscles, and then the stability training of pelvis, hip joint, lumbar spine and core.

Finally, three typical cases handled in the past are introduced.

0 1 Bend over and get groin pain

This is a case last August. A boy who often works out and has no obvious injury suddenly finds pain in his left groin while doing squats and hard pulling.

At first, I regarded it as muscle injury, stretching, massage and acupuncture. It improved after that, but it still didn't work the second time.

Later, I tried to help the left pelvis lean forward and found that the symptoms basically disappeared. This means that it is a bone and joint problem. However, at the time of examination, there was no abnormal dislocation of both pelvis. What's going on here?

My analysis is that the pelvis and hip joint are unstable, or that the "roll-slip" mechanism of hip joint has obstacles. After I hold it, the pelvis can remain stable, and the acetabulum slides forward on the femoral head and rolls forward at the same time.

After finding out the reason, the treatment was simple, the joint was loosened and stabilized, and it returned to normal after 5 times.

Anonymous abdominal pain exceeded in 2002.

This is the case in 2020 10 month. Female teacher, the right lower abdomen hurts for many years, not all the time, nor is it severe, but the attacks are irregular and frequent. I didn't find any problems when I did B-ultrasound CT.

After examination, the pain site is not very clear, probably in the lower two fingers of the anterior superior iliac spine. As there was nothing wrong with the internal organs, I examined the muscles and soft tissues and found that the problem was not serious. Finally, I thought of the lumbar plexus. Sure enough, the pain disappeared immediately after treatment and recovered after two consecutive treatments.

03 sedentary groin pain

This situation is very common and often encountered. Mainly sedentary causes muscle strain and poor blood circulation.

It is right to consider and deal with it from the perspective of muscle and fascia chain.
